Geraldine M. Michalak (Zalesny) of Chicago, Illinois, age 75, has passed away at her residence in Rolling Meadows under the loving care of her family. She is survived by her two daughters Victoria (John) Conway and Debbie (Dan) Pokorski; one granddaughter Hailey Pokorski; one sister Gloria (the late Al) Jumbeck. She was preceded in passing by her beloved husband Jerome, son Brian Michalak and her parents John and Helen Zalesny (Iwanski). Memorial visitation to honor the life of Mrs. Geraldine Michalak will held at Kolbus-John V. May Funeral Home, 6857 W. Higgins Ave., Chicago on Friday, January 2, 2015 from 4 to 9 P.M. and Service will be at 5:30 P.M.In lieu of flowers, memorial can be made to American Cancer Society, www.cancer.org.
