Brian S. Michalak of Chicago, age 50, U.S. Army, Gulf War Era, is survived by his mother Geraldine nee Zalesny; two sisters Victoria (John) Conway and Debbie (Dan) Pokorski; niece Hailey Pokorski. He was preceded passing by his father Jerome Michalak. He enjoyed watching movies and an avid collector of many things. A memorial service to honor the life of Mr. Brian S. Michalak will be held at Kolbus-John V. May Funeral Home, 6857 W Higgins Ave., Chicago on Saturday, November 15, 2014. Family will receive friends for visitation from 4 to 9 P.M. Service 7:30 P.M.
